3. A copy of Sale Tax Certificate of applying business Concern (If Applicable).

PERSONAL

1. A copy of CNIC of the Proprietor.
2. Two (02) Passport size photographs of the Proprietor.

BUSINESS

1. Original bank Account Maintenance Certificate of applying firm.
2. A Copy of Lease deed; Allotment letter; Rent Agreement; or Ownership Documents (whichever is applicable) of the office on the indicated address of applying business concern.

2. A copy of Taxpayer Registration Certificate of all the partners of the applying firm.
3. A copy of 181 Order to Grant of all the partners of the applying firm.
4. A copy of Sale Tax Certificate of applying firm (If Applicable).

1. Copy(s) of CNIC of all the Partner(s).
2. Two (02) Passport size photographs of the Representative (Any Partner).

BUSINESS

1. A copy of Partnership Deed.
2. A copy of Form “H”.
3. Original bank account maintenance certificate in the name of business.
4. A Copy of Lease deed; Allotment letter; Rent Agreement; or Ownership Documents (whichever is applicable) of the office on the indicated address of the business Concern.
